  1. Implant + capsule removal — using en-bloc or total capsulectomy as indicated
  2. Tissue assessment — surgeon evaluates remaining breast tissue, skin envelope, nipple position
  3. Mastopexy pattern selection — vertical (Lollipop), Wise pattern (anchor), or periareolar based on degree of ptosis
  4. Internal reshaping — auto-augmentation techniques may use the patient's own tissue to add upper-pole fullness
  5. Nipple-areola repositioning — typically pedicled to maintain sensation and blood supply
